About this Program
The Urban Initiative Taking Initiative Center is located at 514 Whalley Avenue
in New Haven. The program works with people in New Haven who have not been
reached by our addiction service system, as well as those who have tried
treatment and had been unsuccessful. The Urban Initiative, a unique alliance of
networked agencies, seeks to reach these people so that balance and stability
may be restored to their lives. The aim of the program is to engage, motivate
and support those served so that they can adopt a healthy daily routine, gain
employment, and reconnect with lost relationships and opportunities. Outreach:
Urban Initiative outreach workers are in the community seeking out people who
have not had success with previous recovery attempts or who have never tried to
begin recovery. Many outreach workers are in recovery themselves and use their
experiences to help draw people in for services. The outreach workers strive to
help those long disillusioned with the "system" realize the resources now
available to them. The Taking Initiative Center:
The Taking Initiative Center is a place to foster dialogue about what the next
steps may be for recovery. It is also a place where people can safely talk with
their peers, consult with counselors, and just get away from their normal daily
routine. Individuals can make use of free laundry facility, coffee shop,
computer classes, recovery groups and extensive literature. Access to Treatment:
If immediate transportation to treatment is needed the staff will assist you to get there.
Eligibility:
People residing in New Haven who are not actively involved in the service system
and who have not had success with previous recovery strategies. People will be
screened for DMHAS eligibility.

Supportive Housing Vocational Program

"A Comprehensive Approach to the Achievement of Economic Self Sufficiency"
About this Program
The State of Connecticut Department of Mental Health and Addiction Services
funds Supportive Housing Program (SHP) and Outreach and Engagement Program are
located at 526 Whalley Avenue in New Haven, CT. These programs provide services
to homeless adults with mental illness and/or co-occurring substance abuse
disorder. The Outreach and Engagement Program staffs identify and engage
homeless individuals who may be eligible for services. Outreach and Engagement
staff provide intensive case management services, including linkages to clinical
and employment services.
SHP vocational staff members work with program participants to identify needs,
goals, and resources to maximize independence. The individual plans and services
are based on each individual's vocational goals. Vocational staff collaborate
treatment planning with clinical, residential, and social programs to achieve
established goals. While the overall goal is career development for the purpose
of achieving economic self-sufficiency, the program takes a broad approach to
addressing and removing barriers to employment such as lack of housing,
immediate health conditions, addiction, etc.
The program offers vocational interest inventories/career planning, job seeking
skills instruction, job development and follow-up services. Also available are
independent living skills training, job performance evaluation, case
coordination, referral services and individual counseling. Services are
continued based upon the individual's need and their expressed desire to work.
To insure that the dignity and respect of each individual is maintained and
promoted throughout the community and that people have the opportunity to assume
positive adult roles, staff seeks opportunities for consumer choice and
empowerment. The program also seeks feedback from program participants regarding
satisfaction with services.
Eligibility:
To be eligible for the SHP and its various components, individuals must be
homeless, age 18 or older, reside in Catchment Area 7 (New Haven, Hamden and
Woodbridge, CT) and have a mental illness or co-occurring substance abuse
disorder.
