What We Do
In-Home Services provides supports to individuals who are living in their family home, as well as to people living in their own apartments.
Services focus on encouraging the use of existing natural supports and fostering involvement in social and community activities while providing intensive, rehabilitative community support, crisis intervention, skill building for activities of daily living with a goal of maximizing independence. Staff will become intimately familiar with the participant's surroundings, strengths and challenges, and assist the person in learning skills applicable to his/her living environment.
Service include: performing household tasks, providing instructive assistance, cuing to prompt the person to carry out tasks (e.g., meal preparation; routine household chores, cleaning, laundry, shopping, budgeting, bill-paying and participation in social and recreational activities), providing supportive companionship, instruction or cuing to prompt the participant to dress appropriately and perform basic hygiene functions, supportive assistance and supervision of the participant, and short-term relief in the home for a person who is unable to care for himself/herself when the primary caregiver is absent or in need of relief.

Direct Staff Rate: $35.00 per hour
Case Management Rate: $25.00 per hour
